E211: Plasma biomarkers for early-stage MASH-HCC detection

Introduction: The global incidence and prevalence of chronic liver disease and its sequelae ESLD and HCC are increasing. MASH related HCC is the second most common cause for cancer death in the US, affecting preferentially racial/ethnic minorities, veterans, and low socio-economic populations with health access disparities, i.e., the Appalachian population. The outcomes gap may result from the lack of reliable biological markers for early-stage HCC detection. We hypothesized that p<span style="font-size: 11.0pt; line-height: 107%; font-family: 'Arial',sans-serif; mso-fareast-font-family: Calibri; mso-fareast-theme-font: minor-latin; mso-ansi-language: EN-US; mso-fareast-language: EN-US; mso-bidi-language: AR-SA;">lasma metabolic disturbances observed in the progression of MASH to HCC and associated with epigenetic changes and protein expression dysregulation from apoptotic and autophagy activities predict early-stage HCC.                                                                                                                                                                                                                  

Methods: Plasma metabolites and proteins concentration in MASH and MASH-HCC mouse models, and in human subjects were assayed by Liquid Chromatography-Mass Spectrometry (LC-MS/MS) and ELISA, respectively. Statistical differences among groups were detected by ANOVA, or Principal component analysis (PCA) using Rlab.                                                                                                                                  

Results: Metabolite and proteins concentration in mouse models and humans had similar trends and discriminated MASH-HCC from MASH and controls. The plasma level of survivin was significantly higher in patients with HCC vs. MASH vs. controls (p< 0.05, Fig. 1).  Lower glutathione and higher lactate, valine, and isoleucine concentrations were significantly associated with HCC vs. MASH.                                                                                                                                         

Conclusions: Plasma metabolites and protein expression discriminate MASH-HCC.
