12816 patients were identified; 3422 (27%) in the 70-79yo group and 873 (7%) in the 80-89yo (Table 1). There was no significant association between age group 70-79yo, 80-89yo or 90yo+ with major complication compared to age groups 50-59yo and 60-69yo [70-79yo 0.037, 95%CI [-0.11, 0.19]; p=0.6; 80-89yo 0.122 95%CI [-0.13,0.37]; p=0.3]. However there was a protective effect observed for female sex in regards to major complications [-0.27, 95%CI [-0.41, -0.13]; p< 0.001].
There was a slight increase in LOS in the 80-89yo group of 1.1d p=0.002; which was not redemonstrated in analysis of female patients 80-89yo (-0.78d p=0.2). There was a significant increase in total charges in the 80-89yo group [+$14,685 +/- $6726, 95%CI [1499-27869]; p=0.029.]
Conclusions: Short term post-operative complications are similar between age groups, including septuagenarians and octogenarians undergoing pancreatectomy. However, for octogenarians there was a slight increase in LOS and significant increase in total charges- with a protective effect noted of female sex. Further study is warranted, however our findings suggest that pancreatectomy is a reasonable consideration in carefully selected older patients.
